Refer/consultation with other specialists as needed, order laboratory and diagnostic tests, review results and initiate appropriate intervention, prescribe and adjust dialysis-related medications such as those prescribed for blood pressure, bone disease, including vitamin D, anticoagulants-heparin, erythropoietin and iron, adjust and prescribe non-dialysisrelated medications, comprehensive access assessment and management, management of central lines for hemodialysis and peritoneal dialysis catheters, initiate involvement of social services, dietitian, occupational and physical rehabilitation as appropriate, documenUchart notes on all patient activities, provide teaching and counseling aimed at maintenance and prevention of further deterioration in health status of the Chronic Kidney diseases patient. lnstitute a regimen for health maintenance and prevention, provide teaching and education of patient for ESRD in modality of therapy selection, including transplantation and evaluate patient and family social, environmental and home care needs, perform and interpret office laboratory tests such as U/A, blood glucose, hemoglobin and stool for occult blood among others.
